* |
If your child has suffered an injury that requires casts, crutches, sutures, wheelchair or walker, the parent/guardian must contact the school nurse prior to the student reporting to school. |
* |
Parent/guardian must provide documentation from the student's physician identifying any restrictions to the student's regular activities, the weight-bearing status, level of transfers and any safety concerns. |
* |
School administration will inform the parent/guardian of any special arrangements, program modifications, and/or restrictions that may need to be put in place for the safety of your child in the school setting. |
* |
Parent/guardian must provide documentation from the student's physician identifying when your child may resume all regular school activities. |
